In order to work in the state of New York, one must be able to gain entry into the NAR. This can be done by being able to challenge and pass the competency evaluation exams which are given by Prometric. The exams are done in order to determine the competency of the nurse aide candidate in terms of knowledge and skills in taking care of the patients.

Every candidate has a total of 90 minutes to answer all of the questions. All of the questions will be in multiple-choice format with only one correct answer. One can download a sample of a written exam at www.prometric.com/nurseaide. The questions on the sample test are formatted in the same way as of the actual exams. The practice exams however, only consist of 50 questions. Each practice exam costs $10 and can be paid online with the use of a credit or debit card.

The written exam is also offered by the testing centers orally. This is helpful for the candidates who are not that good with reading English. To have an oral exam, the candidate will need to request to take it during the time he applied for the competency exams. Nurse aide candidates who choose to take this test will be given a player and a headset to listen to the questions. The candidates are allowed to replay the questions if they have any trouble hearing them.

The second part of the exams is the clinical skills portion. The nurse aide candidate will be asked to perform 5 randomly selected nursing skills from a generated list of 22 skills that were taught during training. In order to pass this, the candidate will need to perform all 5 skills satisfactorily. These skills will be performed in front of a nurse aide evaluator who will have a list of checkpoints that the candidate must perform in while doing the skills. To receive your CNA certification in the state of New York you first need to attend a state approved training facility. The nursing assistant program you select needs to offer at least seventy-five hours of combined classroom instruction and clinical practice. In the school you will learn basic health care skills such as  CPR, emergency procedures, communication, residents rights, and ethics. Upon completion of the class you will need to submit a completed background check, school transcripts, fee, and application to apply to take the nurses aide exam.

In order to establish eligibility to apply for the exams, one can enroll in and complete a state-certified CNA training program. If a nurse aide candidate is currently employed in a NY nursing home may not be charged for any cost of nurse aide training, which includes the costs for books and other materials, or for the costs of Competency Exams. All the charges will be paid by the State of New York.

When the nursing home is paying for the nurse aide testing, the payment must include the name of the candidates that are testing on that day. Prometric does not accept any future payments of sent applications that do not include any payment. The payments must be made in the form of a money order, company check, or a certified checks that is made payable to the state’s Commissioner of Health. Personal checks and cash are not accepted.

There are also other ways to gain reimbursement for nurse aide training. If an individual who paid for his own training and competency exams gets hired in a nursing home within 12 months from completing the training or date of testing, he may request reimbursement from the nursing home. It is important that the individual keeps all the original receipts he got from the training.

The nursing home will then take the receipts and mail their copies to the New York Department of Health. It will be the state that will issue the payments to the nursing home, which will be used to reimburse the nurse aide. Although there are several different routes to get a New York CNA certificate, there are only two general ways to gain entry into the state’s NAR. One is to take the competency evaluation exam, which is for those who completed a training program, nursing graduates, and foreign registered nurses; while the other is directly applying to gain entry into the Registry. The latter one is for those who are already nursing assistants from other states who are applying for reciprocity and for U.S. RNs and LPNs who want to become a nurse aide.

Every applicant is allowed to take the exams up to three times. After failing for the third time, the candidate will need to undergo retraining before he will be allowed to retest. When retaking, the candidate will only need to take the exam that he failed in.

An initial CNA certification that was issued in New York is only valid for up to two years. Every certificate has its expiration date on the last date of the month that it was issued on. For example, if a nursing aide certificate was issued on the month of March of 2012, its expiration date will be on March 31, 2012.

The notice serves as a reminder and is usually mailed about 45 days before its expiration date. The Registry will mail the notice on the mailing address that is on the nursing aide’s records. This notice however is not important in the recertification process.

In order to become eligible for recertification, the nursing aide must have worked at least 7 hours as a paid nursing aide for the past 24 months. According to the state regulations, it is the job of the nursing aide’s employer to process recertification and pay for its fees. If the employer is not approved by the NYSDOH as a nursing aide employer, the work experience during that time will not be honored and the nursing aide may not be eligible for recertification. If a certificate is not renewed before its expiration date, it will be considered lapsed. A lapsed certificate can only be renewed by either retesting, or retraining and retesting. This will depend on which application route that the nursing aide followed to gain entry into the Registry.

If there are any changes in the personal information of the nursing aide, employers are encouraged to remind their employees to inform and update the Registry. These changes may either be a change in name, address, contact information, or employer. Once the certificate is renewed, the new information will be placed on the certificate. The nursing aide can also request for a duplicate copy of his/her certificate by paying the appropriate amount.

Nurse aide applicants wanting to work in a nursing facility in the state of New York are required to be certified if they want to work. This can be established by either directly applying for the NAR or taking the license exams.

The exams consists of written/oral tests and a clinical skills tests. Another way of getting into the registry is by applying for reciprocity. When applying, the applicant must have a current and valid license. He must also provide proof that he completed the necessary training and testing that meet both his state and the federal government’s requirements. A part of the reciprocity process is the background check for criminal records, the applicant must not have any records that may disqualify him from becoming a nurse aide in the state of New Mexico. The Disqualifying convictions include:

Some of them will be common such as refraining from touching the patient with dirty hands and the proper removal of used gloves.  There will also be some that will be specific for a certain type of procedure.

This test will be done by either a physician or an advanced practice nurse no less than two weeks before the start of duty. There are some facilities that will do the test immediately upon hiring. If it was done upon employment, the examination may be deferred up to 30 days before the start of work.

This test is not only limited to nursing assistants but is a mandatory screening for all new employees. The test will be a two-step Mantoux tuberculin skin test that includes 5 tuberculin units. The only employees that are exempted from this test are those that got a negative on a two-step Mantoux skin test within the last year, those who got a positive in the same tests, and those who were able to get the necessary treatment for tuberculosis.

A Mantoux test determines whether a person has tuberculosis or not. The test if positive if the results yield at least 10 millimeters of induration.   If positive, a chest x-ray will be done to support the findings and if necessary, this will then be followed by chemoprophylaxis.

An employee with a positive result will be referred to either his personal physician or to the hiring facility’s contracted physician. If the tuberculosis is active or at least suspected to be, he will be asked to refrain from work up until the physician deems it fine to go back.

The Directors of Nursing has viewed this law as an extended requirement to the students who will also handle the patients during the clinical portion of their training. As a result, nursing assistant candidates are also required to undergo this testing. The test will be done before the student starts class.

In New Jersey, there are two types of nursing assistants. Each type has its own CNA training program that you are required to enroll in if you wish to be a CNA in the state. Once you complete a program, you can then apply for the license exams that are similar to the type of program that you enrolled in.

Some may aim to get administrative certifications in work as management in a nursing care home, or some may reach for advance certifications like becoming a medication assistant.

In order to be eligible to get a medication assistant certificate, you must be a current and active nurse aide, home health aide, or a personal care assistant that is certified in New Jersey. For those who want to become a nursing home administrator, there is also an additional training course/seminar that must be completed before one is able to apply for the licensing exams. The course/seminar last about 100 hours and will consists of different lectures that are related to long-term care administration. In addition to that, they will also be required to complete an Ethics Program that is specific to nursing home administrator candidates. The program may be taken online. As for the licensing exam, this will be a computer-based test that is administered by the Professional Examination Services through the National Association of Boards of Examiners of Long Term Care Administrators.

One of the requirements to be able to work in the state of New Jersey as a nurse aide is that you must have a license. You can only get a license if your name is on the registry and that you have passed the state nurse aide license exams. The type of exam you will take will depend on the type of CNA training program you enrolled in.

A nurse aide candidate who was employed by a licensed healthcare facility prior to enrolling in either a NATCEP or PCA training program may not be billed for any fee for the training. All the fees such as payment for books and other materials that will be used in the course of the training will be paid by the hiring facility or employer. In the event that the candidate is fired from the facility, he may still not be billed for the payments.

There are also other conditions that can allow a nursing aide to get reimbursement once he gets work. If a nursing aide has not received any offer for work within 12 months from the day he got his name entered on the Registry, he will be eligible for reimbursement of a reasonable cost from his first employer. The reimbursement will be on a pro rata basis.

To avoid any confusion, the training facilities, like community and private vocational schools, technical schools, and high schools should advise their students to keep the original receipts and a copy of them. These receipts should be presented to the employer upon hiring.

When a nursing aide becomes a regular employee, he will be subjected to the mandatory Mantoux testing, which will be done by either a physician or an advance practice nurse. This test must be done within two weeks before the first day of work. The Mantoux testing is done to protect the patient’s from communicable diseases. This test determines if the individual has TB or not.  If an employee has tested positive in the tests, he will be immediately referred to a physician. If the TB is active, the employee will be asked to leave work until the physician tells it is okay to go back.

For you to be a nursing aide in New Jersey, you must first complete a CNA training program. PSI is an independent testing company that was authorized by the NJDOH to help in the administration, scoring, and the reporting of the results. It is also in charge of running the Nurse Aide Registry of New Jersey. To make a reservation, the candidates will need to contact (800) 733-9267. They can download a copy of the application forms, as well as a copy of a NA Practice Written Examination sample. If the candidate has trouble with written English, he may ask for an Oral exam instead. The testing centers will also accommodate those that request to have special considerations for certain disabilities.

For those that are applying for reciprocity, their certification process will be entirely different, except for the mandatory criminal background investigation that is required for all nursing aide candidates. When they apply, the applications will need to provide the Department of Health proof of their certification. The applicants must also not have any records of resident abuse or neglect. All the necessary forms will be provided in the Reciprocity Application Packet, which they can get from PSI.
