Taking the license exams is one of the ways to gain entry into the New York State Nurse Aide Registry. The exams consist of a written and a skills evaluation on the basic nursing skills. An individual may also request to have the oral examinations instead. The candidate may only take the exams in an approved testing center which may either be at a Regional testing site or a training facility that was approved to conduct in-facility testing.
If a training program also wishes to conduct the license exams in their own facility, they will need to apply for in-facility testing first. This application form will be used as a cover page with the batch of candidates who wish to take the tests. The payments for the testing fess should also be included during the submission.
The application forms must be received by Prometric no less than 10 business days from their scheduled tests. It will be the responsibility of the facility to inform their candidates regarding the schedules. If the candidate is absent or late, he will no longer be allowed to take the test and will not receive any refunds for the payments.
Applicants can download a copy of the application forms at: https://www.prometric.com/en-us/clients/Nurseaide/Documents/New%20York/NYNARequestforInFacilityTesting20111104.pdf.
To ensure that the delivery of the application forms be received in a timely manner, it is not recommended to use USPS certified mail. Aside from mailing the applications to the address provided, they may also be faxed. All materials must be sent to:
Prometric,
NY NA IFT Processing,

Most of those who take in-facility training are currently employed in a nursing facility. For those who aren’t employed or are working in a facility that is not allowed to conduct the exams will have to apply to take the exams in a regional testing center. These individuals will have to submit their applications and the payment fees directly to Prometric. Prometric will then be the one who will give the schedule when the candidate may take the exam at the testing center that was indicated on the application form.
