One of the tasks of the state NA Registry is to keep track of the workers that are on the list. This involves having up to date important information. That is why CNAs are required to fill out the NA Update form if they have any changes or additional important information to their life.
Examples of information that must be reported to the Registry are changes in names or addresses. When submitting the Update forms, the individual will be required to provide his or her current address and social security number. The state uses an individual’s security number as his or her certificate number.
For changes in address, the individual may directly contact the Registry at (651) 215-8705 or call their toll free number at 1-800-397-6124 in order to inform them. These calls should be done during working hours which are between 9 in the morning and 3 in the afternoon.
